2024 Championship Prize Winners & Travel Reimbursement

Thank you for participating in the Donor Games 2024 Championship!

The National Kidney Registry will make all payments to athletes via ACH payment. This may include travel reimbursements as well as cash prizes for the Championship. Please use the form below to provide your bank and account information for payment. If you are submitting travel reimbursements, you will also submit your receipts and related travel documentation through this form.

Because most athletes will be eligible for a 1099 form based on these payments, we also ask all athletes to provide their SSN# and mailing address. By providing this information, the National Kidney Registry will distribute 1099 forms for 2024 in a timely manner.

Please complete the form below by September 21, 2024 in order to receive reimbursement. All prize distributions and travel reimbursements will be made by the National Kidney Registry on Wednesday, September 23, 2024.
